This normally means that the cardholder did not complete the transaction in time or did not complete the final verification step.
This could be due to the card not being 3D secure (mostly overseas cards which have not been activated by the bank for South African online shopping. They need to contact their bak to activate 3D secure on the card)
If they do not have 3D secure on their card, the cell number they put in will not be linked to the card and they will not receive an OTP pin or bank verification request.
